Oh, it's just so sad to hear about your friend's story. I can not imagine someone can love his or her cousin, if that happens to me it'll be sure the most terrible thing I've ever been through. But I hope your friend can make a right decision to get over this problem. I think telling the parents is not a good thinking. It's hard to understand such a relationship. I think when the cousin comes back, she and him, if possible you also should talk about it and the best thing is they will try to stop feeling for each other. I think that this is their own problem, so try to fix it, if it can't work out, only them hurt is enough, don't make the parents suffer because of them. That's what I think, hope that'll help ^____^
